Which measurements support laser suitability, and what is my corneal ectasia risk?
AAO EyeWiki says ectasia screening needs tomography, not just central thickness or a front-surface map. Abnormal topography or keratoconus is an absolute reason not to perform LASIK. A residual stromal bed of 250 micrometres has been proposed as a LASIK threshold, although no single number excludes ectasia; some authors use 300 micrometres more cautiously. With normal topography, a percent tissue altered of 40% or more—flap thickness plus ablation relative to corneal thickness—is associated with higher risk. Do I have dry eye or an ocular-surface problem, and should it be treated before laser?
AAO material treats uncontrolled dry eye, blepharitis and atopy as barriers; lid-margin disease should be treated before surgery because an unstable surface makes measurements less reliable. The FDA warns LASIK can worsen dry eye and that it can be permanent. Marked but controlled dryness may favour surface ablation or SMILE over flap LASIK, but that does not remove the risk. Why is LASIK, iLASIK, PRK, TransPRK or SMILE proposed for my eye?
LASIK creates a flap. AAO says people in combat sport, military service or work with eye-injury risk should consider PRK, other surface ablation or SMILE because a flap can be displaced for life. A thin cornea or ectasia risk can rule out LASIK; the FDA warns refractive surgery on thin cornea can cause complications including blindness, and contact sport can affect candidacy. “iLASIK” or “no touch” does not replace this comparison. What is the visual target, and what applies if prescription remains or enhancement is discussed?
The FDA says only some patients achieve 20/20 without glasses, under-correction and over-correction can occur, and enhancement is not always possible. Glasses or contact lenses may still be needed. “20/20 or money back” is not a medical guarantee. If vision suddenly drops, pain is severe or infection is suspected after I return home, what is the emergency route and who pays?
The NHS says a sudden increase in floaters or flashes, a curtain or dark shadow, or sudden blur needs urgent eye assessment because retinal detachment is possible. In the UK, NHS 111 can provide urgent advice; elsewhere, use the local urgent eye service or emergency department. Redness, pain and light sensitivity need same-day eye assessment. The FDA notes that a displaced LASIK flap, inflammation or infection can require another procedure and intensive drops, with vision loss temporary or permanent despite treatment. Before overseas care, obtain the cost of further treatment and return travel in writing; travel insurance usually excludes surgical complications.
