Why am I a candidate for this weight-loss procedure, and how do other illnesses change the choice?
Diabetes, sleep apnoea, blood pressure, medicines, earlier treatment and the realistic ability to maintain follow-up should be recorded for the individual. Sleeve, bypass, balloon and ESG require different assessments. ASMBS describes ESG as an endoscopic option for selected adults who have not achieved enough loss with lifestyle measures and medication, are not ready for surgery, or have greater surgical risk. Most supporting data concern BMI 30–40; a plan outside that range needs its own rationale. How do reflux or hiatal hernia change the choice of sleeve, bypass, ESG or balloon?
Marked reflux or a hiatal hernia should not be handled as though sleeve, a volume-restricting balloon and ESG pose the same question. A sleeve can worsen reflux; severe reflux may lead to a separate discussion of bypass. Reflux symptoms and, where indicated, endoscopy should be documented before choosing a balloon or ESG. This does not mean every reflux case will resolve with bypass.
How does endoscopic sleeve gastroplasty differ from a surgical sleeve for my situation?
ASMBS describes ESG as a volume-reducing procedure that preserves the gastric fundus and slows stomach emptying. It should not be presented as having the same weight-loss or metabolic effect as a surgical sleeve or bypass. MERIT reported mean total body-weight loss of 13.6% at 12 months, while published series have reported 15–17.5%. The sutures and anchors can make a later sleeve, bypass or duodenal switch more difficult and increase its risk. If an urgent problem or nutritional deficiency appears after I return home, who follows it and how are costs divided?
The NHS recommends lifelong blood testing for vitamin deficiency and, in the UK, 999 for an emergency and 111 for urgent NHS advice; elsewhere, use local emergency and urgent-care routes.
